Please use this form if you need to request an exam proctor while taking a test at the Huntington Public Library. Please note the following:

A separate request form must be submitted for each course / exam requiring a proctor.
All proctoring appointments must be made in advance. We will reply to you within 72 hours to confirm your appointment request if we have availability.

The proctor will take submission of this form as an indication that the student is aware of and willing to comply with all Huntington Public Library Proctoring Guidelines
